Download Form (PDF)*Important Note: The Department of Educational Leadership and Policy Studies does not provide travel reimbursement or meals for prospective students who visit outside of the invitation-only Interview Days (some meals are provided during Interview Days). Additionally, please try and provide multiple options for your visit as faculty and students are in and out and may not be available to meet with you if you only provide one day and date.
